Community Health Worker – St. Mary’s Health Clinics

80 hours/PPD

For more than two decades St. Mary’s Health Clinics (SMHC), a ministry of the Sisters of St. Joseph, has provided free primary health care to the uninsured in the seven county metropolitan area of St. Paul, Minneapolis and their surrounding suburbs.

We have an opening for a Community Health Worker. The responsibilities include providing quality bilingual, bicultural health education, screenings, advocacy, and referrals to community agencies and medical providers. Additionally, this person will disseminate health information through one-to-one educational interventions as well as group presentations at various community sites and in SMHC clinics.

QUALIFICATIONS:

  • Certification in Community Health Worker.
  • Equivalent combination of related education and experience may be considered.
  • One year experience in public health/community health, nursing or related field preferred.
  • Fluency in written and spoken English and Spanish and experience translating from English to Spanish.
  • Excellent interpersonal skills and self-motivated.
  • Ability to work independently and as part of a team.
  • Knowledge and experience with direct local patient services.
  • Must be able to meet the physical demands of the position which may require lifting, pushing, pulling, stooping, bending, reaching, walking and standing for short periods of time.
  • Must be able to work some evenings and weekends.

Clinical Nurse Manager – St. Mary’s Health Clinics

40 hours per week

Mission and Ministry:

The mission of St. Mary’s Health Clinics, in the tradition of the Sisters of St. Joseph of Carondelet, is to carry out the healing ministry of Christ by providing needed health services to the medically underserved or uninsured.

Position Summary:

The Clinical Nurse Manager will provide ongoing assessment, coordination, supervision and support managing the clinical operations and development of St. Mary’s Health Clinics and/or related medical programs. Actively coaching and engaging staff while modeling expected behaviors and holding staff accountable. Evaluating clinic performance to identify, implement and meet quality and safety outcomes. Manages processes to improve patient, employee and provider satisfaction. Manage the clinic budget to meet objectives. Identifies and evaluates barriers to clinic success, helps craft solutions and implements improvement plans. Works to maximize the flow of information between the clinics, the volunteer staff and central office. Assists leadership in achieving coordination, consistency and efficiency of clinics and/or related medical programs. Continuously focuses on reducing non-value added tasks to improve the overall efficiencies of the clinic operation and patient centered care.

QUALIFICATIONS:

  • Registered Nurse with current license to practice in the State of Minnesota
  • Minimum three years’ work experience in medical/surgical nursing – clinic or public health experience preferred.
  • Three-years’ experience in supervising professional and para-professional medical personnel is required
  • Demonstrated ability to work independently and within a team structure is required.
  • Strong organizational and communication skills are required.
  • Demonstrated problem solving skills as evidenced in adaptability and a focus on strategic operations
  • Requires valid Minnesota driver’s license.
  • Demonstrates respect for diversity.
  • Awareness of psychosocial barriers that can impact lives of uninsured.

Sarah’s… an Oasis for Women Overnight Onsite Staff – A

20 hours a week
Monday, Wednesday, Thursday, and Friday from 10 p.m. to 3 a.m.

Sarah’s… an Oasis for Women (Sarah’s) provides housing, community and safety for women in transition. Through in-home support and life skill development, residents are empowered to restart their lives and find healing and hope. Sarah’s is located in the Highland Park neighborhood of St. Paul. The responsibilities of the Overnight Staff focus on security, ensuring a consistency, and a peacemaking presence in the home during nights and weekends. The Overnight Staff is accountable to both the House Manager and the Director, with primary reporting to the House Manager. Working collaboratively with residents, staff, and volunteers, as well as independently, is expected.

Qualifications

  • Demonstrated ability and willingness to relate with and respond to conflicts that arise with women of diverse backgrounds, language skills, and life experiences
  • Ability to communicate clearly in various ways including on the phone, in writing, and through email
  • Ability to handle stressful situations with a calm and respectful demeanor and communication style
  • Experience in hospitality, mentoring, peer mediation, or coaching others preferred
  • Ability to meet physical demands of the position: lifting (20-50#), stooping, bending, reaching, walking
  • Commitment to Sarah’s mission, knowledge of CSJ mission and ministry preferred
  • Demonstrated ability to maintain resident confidentiality and appropriate relationship boundaries

Intake Scheduling Coordinator – St. Mary’s Health Clinics

40 hours/PPD

St. Mary’s Health Clinics is currently seeking a Intake Scheduling Coordinator. The role is responsible for determining patient eligibility, scheduling patient visits for all clinic sites, directing non-eligible callers to appropriate providers and assisting with Spanish interpretation at Spanish clinics as needed.

Qualifications:

  • High School graduate. Technical training in a health related field is desirable.
  • Minimum of two years experience in a medical setting as a receptionist or receptionist or patient scheduler or related area.
  • Ability to communicate with patients concerning medical problems. Microsoft suite and/or willingness to learn, problem-solving skills, flexibility and pleasant telephone manner are required.
  • Proficient verbal and written Spanish language skills as well as English required.
  • Valid MN driver’s license required.
  • Demonstrated appreciation of cultural and ethnic diversity.
